Request Images/Reports

For your convenience you may request images or reports by completing the form below.  On-line requests will be followed up within 24 hours of request receipt (Monday through Friday, 8:00 am to 4:00 pm). If you do not receive a response from us, please contact our office at 702-759-8750. 

Instructions:

  • Fax the completed form to 702-598-1094.
  • Pick up requested materials at 2020 Palomino Lane, Suite 100, Las Vegas, NV 89106.
  • Mailed materials require proof of identification faxed with this form.
  • Allow 24 hours for processing.
  • Materials will be held for 48 hours after requested pick up date, after such time, materials must be re-requested.
